EPC/ Chronic Care Plans
The Enhanced Primary Care (EPC) program, now commonly known as the Chronic Disease Management (CDM) plan, is a Medicare-funded initiative in Australia designed to support individuals with chronic or complex medical conditions.
As of August 1st 2025, Evergreen Clinical Nutrition offers subsidised consultations to patients with current plans.
This service aims to improve the management and coordination of care for patients with chronic illnesses, such as diabetes, mental illness, metabolic syndrome, IBS, IBD, or other conditions lasting six months or more that would benefit from or require multidisciplinary care.
Patients are assessed by their General Practitioner (GP), who determines eligibility based on the presence of a chronic or terminal condition requiring ongoing management.
The GP creates a care plan outlining healthcare needs and goals, including referrals to allied health professionals such as physiotherapists, dietitians, podiatrists, or psychologists.
Why does this matter?
Evergreen Clinical Nutrition provides patients with Enhanced Primary/ Chronic Care plans integrative treatment options they would otherwise be unable to access, which are distinct from traditional dietetics.
Patients with these plans are provided with subsidised services focusing on personalised & therapeutic dietary, nutrient and lifestyle interventions.
The “Gap only payment” service means that a patient with a EPC/ Chronic Care plan who does not have private health fund coverage, is provided with quality primary nutrition health care at a reduced fee.
